Doctor's Choice Health Products (DCHP) explicitly asks when it needs information that personally identifies you ("personal information"). We will collect such information when, for example, you request information about our services, you enter a sweepstakes or respond to a survey on the DCHP site, or you subscribe to a DCHP service. Personal information we collect can include your e-mail address, name, address, telephone number, and credit card number, as applicable. DCHP also may ask for demographic information.
